Job Description

The Healthcare Navigator, who will also function as a Spanish Interpreter is responsible for thecoordination of healthcare services through interdisciplinary collaboration to achieve optimal patient outcomes. This includes facilitating communication, scheduling and managing appointments, guiding patients to the appropriate area of care and/or service, helping patients to understand billing information, and working collaboratively with the organization's clinical/non-clinical personnel to identify barriers, find solutions to create a seamless experience for referred patients. Position demands a highly customer service oriented professional to communicate with patients requesting services within the organization with the goal of eliminating disparities, capturing referrals, exceeding the expectations of the customer and promoting advocacy through effective relationship management.

  • High school diploma or equivalent.

  • Six months experience working in a role requiring medical interpretation.

  • Excellent verbal and written communication skills in English and Spanish.

  • Complete fluency in language(s) for which hired, demonstrated by passing competency exam with aminimum score of 85%, bilingual interview process and role playing.

  • Bridging the Gap, JCCC, SBA or equivalent required.

  • Ability to acquire and maintain strong knowledge of the organization and community health related resources and services.

  • Ability to present a calm, professional demeanor in a demanding, high stress environment, consistentlydemonstrating good judgment and superior customer service and problem solving skills. Handlepatient issues with tact and confidentiality.

  • Excellent interpersonal skills with ability to work with a wide range of people from diverse cultural,socio-economic and professional backgrounds. Autonomous self-starter and team player.

  • National certification preferred.

  • Hospital registration and/or scheduling experience preferred.
